| 内容記述 | [目的] 保健医療福祉の基盤整備の弱い一小離島において、過去の在宅ターミナルケアに関わった支援者たちの支援内容から、「在宅死」の実現要因を探り、看護職者の役割機能をみいだすことである。\n[方法] 対象は、沖縄県南大東島で2000年～2006年(7年間)に在宅ターミナルケアを受けた8事例の支援者たちである。方法は、事例を確定し、保健師記録を基礎に支援者たちに支援内容について面接を行った。支援内容は支援者毎に整理し、その代表的な内容を表示し（小分類）、看護の役割機能である「直接ケアによる支援的役割機能（直接ケア）」、「調整的役割機能（調整）」、「教育・指導的役割機能（教育）」、「相談・支持的役割機能（相談）」の４つで大分類した。\n[結果] 小離島における「在宅死」の実現要因は、(1)行政を含め島の保健・医療・福祉の専門職による参加型で支援すること (2)専門職以外の島民の参加や島外の専門性を活用すること (3)専門職はニーズ重視のサービスを提供すること (4)医師の医療技術指導の下で専門職は柔軟な発想で多機能に支援すること (5)専門職間の双方向での調整や相談、必要時に教育が行われること (6)看護職の看護の4つの役割機能（「直接ケア」、「調整」、「相談」、「教育」）の発揮であった。\n[結論] 小離島における在宅ターミナルケアの実現に向けた看護職者の役割は、看護職の看護の4つの役割機能を発揮することであり、特に「調整役」が求められていることが示唆された。|[Objective]The purposes of this study are to search for the factors which make “dying at home” possible and to find the nurse’s roles in end of life care at home, by analyzing the practices of care givers who took parts in end of life care in a little isolated island with poor infrastructures for health and welfare. \n[Methods] Subjects are the care givers who took parts in end of life care at home for eight cases in Minamidaitou Island in seven years, from 2000 to 2006. Methods are confirming cases of end of life care and interviewing the care givers about details of the practices based on the records of public health nurses. Classified the practices of each care giver, and indicated the representative ways of care of each practice(small categories), and grouping them according to the fournursing-role-functions, which are “Supportive role function by direct care(Direct care)”, ”Coordinative role function(Coordination)”, “Consultative/supportive role function(Consultation)” and “Educative/directive role function (Education)” . \n[Results and discussions] The factors which make the “dying at home” possible were (1)supports by the all health, medical and welfare professionals in island, including the staff of municipality, (2)attendance of non-professional habitant in island and good use of out-of-island professionals, (3)attitude of the professional which attaches importance to the needs of each case, (4)professional multifunctional practices by flexible ideas under the medical doctor’s technical guidance, (5)bidirectional “Coordination”, ”Consultation” and “Education” between professionals. (6) full usage of the fournursing-role-functions. The nurse’s roles in end of life care at home, which make “dying at home” possible, is making full usage of the fournursing-role-functions (“Direct care”, “Coordination”, Consultation” and “Education”). It was suggested that “Coordination” was the especially needed role-function. | |||||
