| 内容記述 | 【目的】緩和ケア病棟の看護師のスピリチュアルペインに関する認識と記録の現状やスピリチュアルペインアセスメント・シート（以下、シートと略す）の導入による看護師への影響を明らかにすることである。【方法】調査対象はＡ病院の緩和ケア病棟看護師11名で、方法は、シート導入前、シート導入中（３週間)、シート導入後の評価を３段階で質問紙調査、個人面接、カンファレンスなどの参与観察でデータを収集した。分析方法は、記述統計と質的帰納的分析で行った。【結果】対象者の平均年齢は、34.9±5.6歳（範囲：28-43)、緩和経験平均年数2.0±1.7年（範囲0.5-5）であった。緩和ケア病棟の看護師のスピリチュアルペインに関する認識と記録の現状として、【時間がかかる】【スピリチュアルペインの難しさ】【記録の重要性】などの６つのカテゴリーが抽出された。シートを活用した看護師は３名で、４事例の患者の記載が見られ、さらにシートを用いたカンファレンスによる情報共有が行われた。シート導入後の評価では、シートの使用期間が短期間であり、シートを活用しての看護実践までは至らなかったが、シートによる看護師に対する意識の向上、自己学習への強化に繋がった。【結論】シートの導入効果として、スピリチュアルペインの意識づけになり、アセスメント能力の向上が見られたことにより、今後も継続したシートの導入は必要不可欠であることが示唆された。|Purpose: The purpose of this study was to clarify nurses' perception and current state of nursing records regarding spiritual pain, also to influence of introduction for the Spiritual Pain Assessment Sheet (SPAS) in the palliative care unit. Method: Eleven participants were surveyed using questionnaires in three steps which were before SPAS introduction, three week of SPAS introduction and after SPAS introduction, also given individual interview and participant observation on spiritual pain conference in the palliative care unit. Descriptive statistics and qualitative induction analyses were conducted. Result: The average age of participants were 34.9±5.6 years old (range: 28-43), and the average years for nursing experience in the palliative care unit was 2.0 ±1.7 years (range 0.5-5). Six categories were conducted such as [it takes time], [difficulty of spiritual pain] and [importance of the record] regarding nurses' perception and current state of nursing records for spiritual pain. The SPAS were utilized by three nurses for four case of patient' spiritual pain. Also conference was held for sharing patients' information. After using SPAS's evaluation, nurses did not intervene for patient' spiritual pain in their nursing practice because the SPAS were introduced on short period (only three weeks). However, using SPAS made nurse's ability to improve their consciousness to patients' spiritual pain and led to strengthen their self-learning for spiritual pain. Conclusion: The introduction of SPAS was an effect for nurses to improve their ability of assessment and aware of their spiritual pain for patient in palliative care unit. In those results, it is necessary to use SPAS in spiritual care continually. | |||||
