| 内容記述 | 【目的】WHOラダーに準じた疼痛緩和のプロセスを行うために、A病院に適したCancer Pain Assessmentツール（以下CPAツール）を作成し、臨床で活用していく中で、どのように普及していくかというプロセスを明らかにすることを目的とした。【方法】がん患者が入院する3病棟の看護師を対象として、研究者がガイドラインなどを参照に作成したCPAツールを臨床に導入し、3カ月のフォローアップを行い、期間中の看護師との関わりを参与観察した。また導入後、計9名の看護師に対して、半構成的面接法を行った。それらのデータを元に質的帰納的分析を行った。【結果】抽出された8つのカテゴリーはツールの普及状況を表す4つの時期に分けられた。普及準備期では〈看護師を含む多職種へのCPAツールの広報不足〉〈CPAツールの使用方法の不統一と戸惑い〉という問題があった。普及促進期では、〈CPAツールを普及させるための病棟での取り組み〉が行われた。普及分岐点ではCPAツールが普及した病棟では〈看護師が使用して感じたCPAツールの有用性の実感〉があり、普及しなかった病棟では〈CPAツールの有用性を実感できなかった背景〉があった。浸透期では〈CPAツールの活用で生じた症状コントロールに対する看護師の意識と行動の変化〉と〈看護師が使用して感じた患者にとってのCPAツールの有用性〉がみられた。継続・発展への課題では〈CPAツールの活用を継続していくための教育の必要性〉が明らかになった。【結論】CPAツールが病棟へ普及していくプロセスは、リーダーやリンクナースなどの推進力となるスタッフとの協働、CPAツールを効果的に活用するための病棟システムの調整やツールの簡便性への工夫が必要であり、看護師がCPAツールを活用して臨床の中で意義あるものとして有用性を実感することが重要なプロセスであった。|【Purpose】The purpose of this study was to explore the utility and dissemination process of Cancer Pain Assessment (CPA)tools that adapted to clinical setting to relieve cancer pain in accordance with WHO ladder. 【Methods】The researcher introduced original assessment tools to clinical nurses, who have been working three wards in a hospital. After introduction, the researcher supported clinical nurses for three months as a participant observation. After three months, data was collected by semi-structured interviews from nine nurses of three wards, and analyzed by qualitative and inductive methods. 【Results】 Dissemination process of assessment tools found four stages with eight categories. A stage of the preliminary dissemination, the wards had problems of “a lack of information of CPA tools for multidisciplinary and includes nurses”, “inconsistency and confusion of how to use CPA tool”. A stage of the promotable dissemination, the nurses of each wards practiced “approach to disseminate CPA tools for nurses on wards”. A stage of the Junctional dissemination, the wards that CPA tools were disseminated had “actually feeling of utility for nurses use CPA tool”, whereas the wards that CPA tool were not disseminated had “background in which utility of CPA tool were not able to be actually felt”. A stage of the dissemination, the wards had “the nurse's action and consideration change into symptom control caused by use of CPA tool” and “Utility of CPA tools for patients who felt it by using”. Subject to continuance and development, the wards explored “necessity of education to continue use of CPA tool”. 【Conclusions】 It was necessary for the process of CPA tools’ dissemination into clinical settings as follows ; first, collaboration with the staff such as the leaders and link nurses who will be the driving force to disseminate CPA tool ; second, adjustment system to make best use of CPA tool, and device of ease for CPA tool. Finally, it was important that nurses who were using CPA tools realize feeling of utility and meaning into clinical settings. | |||||
