| 内容記述 | 本研究の目的は、一度施設入所すると帰省の困難な離島の高齢者に対し、一時帰省（ふるさと訪問）を可能にすることにより、施設入所高齢者の生きがいづくりになることを検証することである。対象は、(1)平成12年度試行的ふるさと訪問を実現した２例、(2)平成13年８月１日現在、竹富町に住民票があり要介護認定を受け、介護老人福祉施設入所中の全高齢者43例、(3)平成13年度にふるさと訪問を実現した７例である。方法は、「ふるさと訪問」事業化への取り組みのプロセスと事業評価、課題を見いだすため、報告者らが意図的に企画段階からAction Research として介入し観察、面接、状況記録での分析、及び介護老人福祉施設入所者には、ふるさと訪問希望調査を質問紙により実施した。結果及び考察：１）２例の試行的ふるさと訪問は、「生きて二度と生まれ島の土を踏むことができない」との思い込みの現状を打破した。２例の反応から、ふるさと訪問は生きがいづくりにつながることが示唆された。２）離島間のふるさと訪問は、陸路と異なり空路や海路が要求され、費用や時間、介護量が課題である。その実現は、施設サービスにゆだねることなく公的サービスにつなげることで実現する。３）ふるさと訪問希望者は、有効回答者の７割であった。ふるさと訪問希望の有無は、性、年齢、介護度、入所期間、世帯構成、主たる介護者、島での生活満足度、主観的健康観、施設での生活満足度、面会頻度、施設での一番の楽しみ、帰省経験との関係はなく、入所経路のみに有意差がみられた。ふるさと訪問希望は、「ふるさとや家族への思い」が影響していることが推察された。４）ふるさと訪問の波及効果は、本人の生きがいづくりだけでなく、家族関係の深まりや施設サービスにも影響することが示唆された。５)「誰もが共に生きられる社会の実現」に向け、行政努力だけでなく家族や地域の理解、安定供給可能な介護ボランティアシステムの構築が課題である。 The purpose of this study is to determine if "home town visiting" for the elderly people, who are from isolated islands and living in care houses, heightens their fulfillment in life. The study subjects are, (1)2 elderly people who experienced “Home Town Visiting” in 2000 ;(2)the 43 elderly people who had resident registrations in Taketomi Municipality at the time of August 1st 2001, and who got the official recognition of Yo-Kaigo-Nintei, and were living in care houses; (3)and seven elderly people who experienced “Home Town Visiting” in 2001. An action research method was introduced. The researchers planned these trial projects, interviewed the subjects, implemented “Hometown Visiting”, evaluated these trials and clarified the problems to be solved in this project. The study method was action research: participatory observation, interview, and analysis. Results and conclusions: 1) The trial of the hometown visiting changed the possessed idea "never can go back to home island alive". The two elders who succeeded in going back to the home island showed positive reactions. The hometown visiting can be regarded as a way to raise the quality of life of the elderly people. 2) The hometown visiting between isolated islands needs more money, time and care, compared to that in Main Island Okinawa, because it requires movements by airplane and ferry over the sea. The successful point of this project is doing this as one of the social services, not leaving it to individual institutions. 3) 70% of respondents hoped for a hometown visiting. The characteristics of the elders and situations before admission had no relationships to the hope. The longing for the sight of ones hometown and family might have influence on the hope for hometown visiting. 4) The ripple effect of hometown visiting on raising of the quality of life of the elders and strengthen the relationships between family members are suggested. 5) The understanding by family and community, the building of volunteer activity system are needed for the society where everybody lives together. |
