| 内容記述 | 手洗い法は院内感染予防において、もっとも基本的であり、看護教育に不可欠である。本学カリキュラムの場合、基礎看護学の中で一過性菌の除去を目的とした「衛生的手洗い法」と、成人保健看護領域の周手術期看護で、高水準な手洗い法として「手術時手洗い」の演習を行っている。成人保健看護領域の「手術時手洗い」演習において、本学３年生82名は減菌効果を確認するためにぺたんチェックスタンプ法を用いて実験を行った。その結果をもとに、「手術時手洗い」の演習方法と結果の評価について検討した。また学生の演習後のレポートからぺたんチェックスタンプ法を用いた学習の波及効果を捉える試みをした。学生82名全員の「手術時手洗い」前後の細菌数を局所的・半定量的に比較した結果は、右手５本の指で明らかに減菌しており、手洗い直後の減菌効果を評価できた。また「手術時手洗い」後における残菌部位は、「衛生的手洗い法」での洗い残しの多い部位と一致していた。演習記録内容は、手指付着菌に関する表現、洗い方・技術に関する表現、手指の状態に関する表現、患者に与える影響に関する表現、「手術時手洗い」の効果および消毒効果に関する表現等をキーワードから捉えることができた。現在の臨床現場は、「手術時手洗い」に関する様々な方法や学説が混在し、一定の手洗い方式が用いられているわけではない。当学内では「手術時手洗い」を学ばせるためにぺたんチェックスタンプ法による評価を取入れているが、手指付着菌を確認し、減菌する方法を理解するために有効であるといえる。また医療現場の現状を踏まえてレベルの違う手洗い法を学習することは、科学的な裏付けをもって、感染防止の基本原則を学ぶために有用であると考える。 To learn the hands washing technique for nurse student is important to prevent the spread of infections, and educational approach is also important to have the students understand it well. To evaluate the educational approach of the surgical hand scrub in our college curriculum, we used the market products "Petan check stamp method" by comparing the bacterial corny counts on fingers before and after the surgical hand scrub. Eighty-two students were stamped their fingers on Petan check before and after hands washing, and the stamp was incubated for overnight. Student counted microbiological colony on the stamp, and reported it. After washing hands, a number of colonies on the right hand fingers of them were reduced remarkably, and they could estimate clearly the effect of the surgical hand scrub. The wash less parts by surgical hand scrub was same position in the hygienic hand washing. The student could understand 5 classified of accomplish level in this educational approach. These results indicate that Petan check stamp method is useful tool to educate the hand washing technique for the student. |
