| 内容記述 | 近年､我が国の障害者基本法が改正され､精神障害者の｢生活のしづらさ｣が生活障害として認定された｡従来の病院収容主義から社会参加への流れとなり､ノーマライゼーションは着実に進展している｡精神障害者小規模作業所は障害者の社会参加を高める為の施策の１つであり､全国にその数は増えている｡ノーマライゼーションの進展には地域の役割が大切であり､市町村への期待も大きい｡しかし市町村の負担も拡大し､特に医療費や補助金等のコスト問題は深刻である｡本研究の対象であるＹ町は､これまで精神障害者への地域活動としてデイケアや相談業務､家族会の支援を行ってきた｡しかし､それに要した町の支出は僅かであり､その効果も十分に評価されていなかった｡2001年に家族会による作業所が設立され､町より110万円の補助金を受けて活動を続けている｡その作業所の経過を踏まえ､コストと利用者の変化を調査し､設立の効果を図る事を目的とした｡ 研究方法 対象者は、沖縄県Ｙ町の作業所を利用する24人である。コストに関しては国民健康保険料の支出に注目し、作業所の設立前後の比較を行った。利用者の変化に関しては、精神障害者ケアガイドライン検討委員会版ケアアセスメント票の「ケア必要度」を用いて、障害の変化を振り返り調査にて分析した。自由記載の欄には、利用者の変化の概要を作業指導員に依頼し、その文章からカテゴリーを抽出し作業の役割を分析した。結果及び結論 １. Y町における｢精神及び行動の障害｣に要する診療費は高額であり､更に医療費が増加する事も予想された｡その為の検討や対策が非常に大切であると示唆された｡ ２. Y町は作業所の設立により､1年目で約260万円､2年目で約330万円の支出を削減し､医療費の抑制に効果を示していた｡ ３. ｢ケア必要度｣の改善には個人差があり､一概に精神障害という同じ疾患でも個別性を踏まえる重要性が示唆された｡ ４. 作業所の機能にリハビリテーションの効果があると示唆された｡ ５. 作業所のリハビリテーション機能の効果として､病状安定とケアの必要性の軽減が挙げられ､その他に現状の能力を維持する効果もあると示唆された｡ ６. 作業所の役割には就労の他､利用者が安心できる場所の提供であり､利用者にとって｢憩いの場｣であると示唆された｡ Backgroud: Normalization of mentally handicapped persons is proceeding steadly in Japan. The purpose of the workshops for handicapped persons is society reversion. The expectation of the workshops on handicapped persons is big but the burden of the municipalities is big, too. The problem of the cost is especially serious. Cost and change in the handicapped persons were examined by this research. \nMethodology: Subjects were 24 people who use the workshops for mentally handicapped persons of Y-town. Health care costs were examined before and after the set up of the workshops. Change in the handicapped person was measured by "the necessity measure of care". Observed changes in handicapped persons were freely recorded by the job mentor and then analyzed by sorting into categories, the role of the workshop was there by analyzed. Results and Conclusion 1. The medical expenses for treating mental disorders in Y-town were large. Future increases are cause for concern as well, and discussion in necessary. 2. Y-town reduced medical expenses by the establishment of the workshops. 3. There was an individual difference in improvement of the handicapped. As for mental disorders, the correspondence which is suitable for that person is important. 4. The workshops for handicapped persons had the function of rehabilitation. 5. Conditions became stable in the effect of the rehabilitation. Conditions didn't become worse additionally. 6. Providing a work role is a part of the workshops for handicapped person as well as a place where they can feel a sense of relaxation and belonging. |
