 note = {【研究目的】離島は、高齢化・過疎化が進み、また隔たった地理的条件によって、島内にある保健・医療・福祉施設が限られていることからくる島特有の課題を抱えている。本研究は、A島で行われている医療の連携、及び専門職者の支援の現状とその全体像を明確化し、円滑な地域連携に繋げるための基礎資料とすることを目的とする。【研究方法】研究対象は定例で開催されている地域連携協議会へ参加している6施設に所属している各専門職者1名に対し半構成的面接を行い、聞き取った内容を逐語録にまとめ、現在の状況、課題、改善点の視点から考察した。また地域連携に関する会議・カンファレンスへの参加を行った。【結果】聞き取り調査で出てくる言葉や話題に着目し、それらが意味するところを要約し、聞き取りの内容を表すテーマをつけ、地域連携時の情報共有、在宅療養支援、要医療処置患者の在宅療養、高齢者の在宅療養、脳卒中患者の在宅療養、緊急時の対応、地域の受け皿に関する内容が挙がった。A島では中核病院を中心に関係機関が問題や情報を共有し患者支援に取り組んでおり、地域連携および在宅療養に関する会議・勉強会が定期的に開催され、円滑な地域連携に繋がっていた。要医療処置患者の受け入れ体制は十分ではなく、急性期病院での長期入院に繋がっている現状がみられた。長期にわたるケアを必要とする慢性疾患患者の経過に合わせて行う支援の充実、および限られた施設数の中で、必要に応じた患者の受け入れが課題となっていた。【結論】地域連携における各職種の特徴的な課題、及び共通の課題があり、地域の利点を発展させながら、課題についての取り組みが必要である。|The purpose of this study is to become the supporting data for connecting with smooth regional alliances by revealing the actual condition and full picture of the medical service's cooperation and support of professional staff at a island. The study considered the aspects of present circumstances, issues, and improvements after conducting semi-structured interview to each professional staff attached to 6 facilities, which are facilities participating in the regularly held Regional Alliances Council, and after literally summarizing the questioned details. Furthermore, meetings and conferences concerning regional alliances were attended. Attention was directed to the words and topics repeatedly heard during the interview; its meanings were summarized; theme reflecting the interviewed contents was given; and matters concerning the following were pointed out: information dissemination at the time of regional alliance, home recuperation support (cancer patients, end stage patients, patients requiring medical treatment, elderly people, and stroke patient), action at times of emergency, regional receiving system. At A island, with the key hospital playing the key role, related organization shared and worked on the problem of home recuperation support, held meetings and study meetings regularly concerning regional alliances and home recuperation, and has connected this to smooth regional alliances. Due to insufficient receiving system for patients requiring medical treatment, presently it is in a state where patients are being connected to long-term hospitalization at acute stage hospitals. It is necessary that support according to the flow of chronic disease patients needing long term care be accomplished. Region's approach is demanded in receiving patients according to their needs within the limited facility.},
