@article{oai:opcnr.repo.nii.ac.jp:00000160, author = {川崎, 道子 and Kawasaki, Michiko and 永吉, ルリ子 and Nagayoshi, Ruriko and 牧内, 忍 and Makiuchi, Shinobu and 大屋, 記子 and Ooya, Noriko and 宮地, 文子 and Miyaji, Fumiko}, issue = {13}, journal = {沖縄県立看護大学紀要, Journal of Okinawa Prefectural College of Nursing}, month = {Mar}, note = {【目的】保健師駐在制を経験した保健師の継承すべき能力およびその継承方法を考察する。【方法】過去に沖縄県で駐在制を経験した保健師(退職保健師11人、現職保健師103人)を対象とし、退職保健師は、訪問での半構成的面接調査を、現職保健師は、無記名自記式質問調査票の郵送による留め置き調査を平成21年1月から2月に行った。調査内容は、保健師駐在制のメリット・デメリット、保健師の継承すべき能力、基本属性等である。【結果】1. 分析対象は、退職保健師10人(90.9%)、現職保健師56人(54.4%)であった。2. 駐在制のメリットについて退職保健師、現職保健師に類似したカテゴリーは、「住民の身近な存在」と「地域に根ざした活動」、「住民・関係機関との連携が容易」と「他機関と活動の共有化・連携」、「現任教育の場」と「職場環境」であった。3. 駐在制のデメリットについて退職保健師、現職保健師に類似したカテゴリーは、「タイムリーな指導が得られない」、「プライバシーの侵害」・「新採用での離島赴任のつらさ」・「急患への精神的負担」・「家族の犠牲」と「単独駐在での負担感」であった。4. 保健師の継承すべき能力には、退職保健師は「住民に寄り添った支援」、「知識・技術」、「個別支援の重要性」、現職保健師は「個人・家族、集団への支援と地域への働きかけの総合的な活動展開」、「地域特性・地域ニーズの把握による地域診断能力・地域アセスメント能力」を挙げていた。【結論】保健師の継承すべき能力は、個別支援および地域診断であることが明らかとなった。それを踏まえ、「沖縄県新任保健師など現任教育育成プログラム」および職場内研修を今後、より強化する必要がある。|[Objective] It is to investigate abilities which have been acquired by public health nurses and methods to pass down the abilities. [Method] The samples are former public health nurses in Okinawa (11 retired health nurses and 103 active health nurses). Between January and February 2009, we implemented semi-structured interviews to the retired public health nurses and anonymous survey (mailed to be called for) to those currently at work. The research contents were about advantages and disadvantages of the public health nurse system, ability of the public health nurses to be taken over, and basic attributes, etc. [Results] 1. Valid responses are from 10 retired public health nurses (90.9%) and from 56 active public health nurses (54.4%). 2. Common answers about advantages of resident system by retired and active public health nurses are: "salience to residents," "community based activities," "easy cooperation with residents and related organizations," "sharing and cooperation with other organizations," "training environment," and "work environment." 3. Common answers about disadvantages are: "no timely training," "violation of privacy," "difficulty in remote islands as the first work environment," "emotional burden to emergency patients," "burden to family," and "heavy responsibility on a solo public health nurse." 4. Abilities that need to be passed down; for the retired public health nurses are: "close support for residents," "knowledge and technique," "importance of personal support," and for the active public health nurses are: "development of total activities for supporting individual, family, and group and for working at community level" and "ability to diagnose and assess the community by grasping community characteristics and needs." [Conclusion] The research explicated that the abilities that public health nurses need to pass down is personal support and community diagnosis.
