 note = {【研究目的】患者の暴力を受けた看護師の体験から、暴力の発生状況及び看護師の状況判断と対処行動を明らかにし、院内暴力への対応策に役立てる。【研究方法】患者からの暴力を体験した看護師で研究に同意が得られた16名の看護師に半構造的面談法による個別インタビューを実施した。【結果】16名の看護師によって語られた23事例を、暴力の発生状況に着眼して4群に分類し、看護師の状況判断と対処行動について分析した結果、各群の特徴を捉えた対応策への示唆が得られた。1群；看護師の言動や対応が誘因で発生した暴力では、看護師の言動が刺激となっているので、患者へのアサーティブな対応の訓練プログラムが必要である。2群；患者の危険行為の制止時に発生する暴力では、看護師の身を危険に晒すような場面が多いので、個人的対処ではなく組織的な対応マニュアルが必要である。3群；処置やケア等の介入時に発生する暴力では、看護師は患者の暴力を病気だからと容認する傾向にあるので、暴力に対する看護師の認識を改める必要がある。4群；性的暴力では、患者に強く言えない若い看護師がターゲットになり易いので、毅然とした態度を示せるようにチーム全体で支えていく必要がある。【結論】患者からの暴力の発生状況の特徴と看護師の対処行動を分析することにより、院内暴力に対する効果的な対応策についての示唆を得ることができた。|Purpose: This study aims to explicate the incidents of patients' violence that nurses experienced and to identify the nurse's coping behaviors. It also aims to utilize the finding for better coping strategies in a hospital. Method: Each of 16 nurses in the hospital was given a semi-structured interview about his/her experience of patients' violence. Results: 23 cases of violence from patients were classified into 4 categories according to the characteristics of incidents. Additionally, nurse's judgment and coping behaviors were analyzed. As a result, we are able to provide practical guides to prevent the violence of each category. 1) Violence induced by the nurse's action and reaction; Assertive training program for nurse-patient relationship is necessary. 2) Violence caused as to intervene in the patient's dangerous behaviors; the organizational guarding system. for patients' violence is necessary.. 3) Violence occurred on bed side during care/treatments; nurse's common recognition that patients' violence is acceptable as it is induced by their diseases needs to be revised. 4) Violence as sexual harassment; team supports are necessary for younger nurses~ who more likely become targets~ so as to encourage them to firmly reject sexual harassment.
