{"created":"2023-06-19T06:41:44.452305+00:00","id":72,"links":{},"metadata":{"_buckets":{"deposit":"e13d8831-b066-4643-8a97-89a8a61a44ce"},"_deposit":{"created_by":2,"id":"72","owners":[2],"pid":{"revision_id":0,"type":"depid","value":"72"},"status":"published"},"_oai":{"id":"oai:opcnr.repo.nii.ac.jp:00000072","sets":["1:10"]},"author_link":["75","118","117","65","211"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2003-03","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"4","bibliographicPageEnd":"109","bibliographicPageStart":"101","bibliographic_titles":[{"bibliographic_title":"沖縄県立看護大学紀要"},{"bibliographic_title":"Journal of Okinawa Prefectural College of Nursing ","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"本研究は一般病棟、訪問看護ステーション、緩和ケア病棟の看護職者を対象にアンケート調査を実施し、沖縄県内におけるターミナル期看護の現状や課題を明らかにする目的で行った。 研究方法：沖縄県内の一般病棟174人、訪問看護ステーション98人、緩和ケア病棟19人の看護職者を対象にアンケート調査を実施した。 結果 １. 一般病棟や訪問看護ステーションにおいてほとんどの施設がターミナル期患者に関する看護基準を有していなかった。２. 沖縄県において、病名の告知、余命告知の割合は全国平均に比較しともに低かった。３. 看護職者は病名の告知に関し、患者が望めば告知は行った方がよいと前向きな考えを持っていた。４. 看護職者は患者が意思決定できるような情報提供は充分ではないが、家族への情報提供は充分に行っていると考えていた。今後は、患者が望めば充分な情報の提供を行い意思決定のプロセスを支え、患者が納得して自己決定できるようサポートしていくことが重要である。５. 看護職者の殆どがターミナル期患者・家族の看護は難しいと感じていた。６. 一般病棟におけるターミナルケアの現状から考える今後の課題として、１)ターミナル期患者に対する看護基準の設定 ２)療養環境の整備 ３)看護職者への緩和ケアの知識・専門技術の普及 ４)緩和ケア専門看護師の養成５）ターミナル期看護に従事するチーム医療の充実であった。７. 訪問看護ステーションにおけるターミナルケアの現状から考える今後の課題として、１)24時間の往診・訪問体制、関係機関との連携 ２)充分な症状緩和、全身状態の管理 ３)家族間の意思の統一への援助 ４)患者、家族が意思決定できるような情報提供であった。\nThe purpose of this study is to clarify issues and current situation in hospice care at home nursing and palliative care wards. Research design: the study was conducted using questionnaire among general hospitals, home nursing stations, and palliative care wards. Findings: 1.General hospital and Home nursing stations don't have standards care for patient in terminal stage of life. 2. It is low rate that doctors inform patient about his diagnosis and expected times patient might have. 3. Nurses have such positive thinking as doctors will tell patient the truth when they want to. 4. Nurses think that they give enough information to help family member's decision making ; however, they think that they do not provide adequate information for patient decision making. We will help patient make his decision, also help the family agree to his decision. 5.Most of nurses have difficulty in caring for patient and family in terminal stage of life. 6.Issues in general hospitals from this study findings are 1)necessary of establishing standards care for patient in terminal stage of life 2)better treatment in hospital 3)knowledge and advanced skills of palliative care 4)foster specialists care 5)better care in the team. 7.Issues in home nursing stations in hospice care are 1)liaison system after office hour, doctors visiting system 2)patient's pain and system management 3)supporting for family decision making 4)providing adequate information for patient and family member's decision
