{"created":"2023-06-19T06:41:44.929263+00:00","id":85,"links":{},"metadata":{"_buckets":{"deposit":"055ec9d0-0a01-474e-89ee-6cd3bf912e5d"},"_deposit":{"created_by":2,"id":"85","owners":[2],"pid":{"revision_id":0,"type":"depid","value":"85"},"status":"published"},"_oai":{"id":"oai:opcnr.repo.nii.ac.jp:00000085","sets":["1:11"]},"author_link":["290","117","65","289","229","211","41"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2004-03","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"5","bibliographicPageEnd":"58","bibliographicPageStart":"51","bibliographic_titles":[{"bibliographic_title":"沖縄県立看護大学紀要"},{"bibliographic_title":"Journal of Okinawa Prefectural College of Nursing","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"本研究の目的は､地理的条件や人口規模等から介護保険制度運用の狭間にある離島におけるケアマネジメントの実態を明らかにし､離島におけるケアマネジメントの課題を検討することである｡対象は､｢沖縄振興開発特別措置法｣に基づく39島中､架橋で陸路が確保されている島や無人島を除く28島である｡方法は､｢高齢者の実態把握及び基盤整備等調査票｣にケアマネジメントの配置形態についての項目を作成し､全離島の市町村介護保険担当職員に送付し回答を依頼した｡28島中､離島のケアマネジメントの類型により選定した２島(Ａ島・Ｂ島) については､地区踏査やケアマネージャーとの面接を実施した｡ 結果及び考察：１) 沖縄県の28有人離島のケアマネジメントは４タイプに類型化された｡島内にケアマネージャーが常駐し､ケアマネジメントを実施している｢島内完結型｣､島外にケアマネジメントを依頼する｢島外参入型｣､ケアマネジメントが選択可能な都市地区と類似した｢一般型｣､自己作成や島内にケアマネジメント対象者が存在せず不要｢その他｣であった｡４タイプの分布状況は､人口規模3,000人以下の小さい多くの島々で｢島外参入型｣が実施されていた｡２) ｢島内完結型｣Ａ島のケアマネジメントは､ケアマネージャーを選択することは困難であるが､常駐しているためケアマネジメント必要時にはいつでも容易に利用することが可能であった｡また､診療所看護師との兼務で医療情報も含めた保健医療福祉の総合的ニーズへの対応の可能性が示唆された｡３) ｢島外参入型｣Ｂ島のケアマネジメントは､天候などに左右され計画的な来島が困難であった｡その結果､アセスメントやサービス計画に先行してサービス提供が優先していた｡介護保険制度のめざす十分なアセスメントに基づく計画的なサービス提供につながらなかった｡４) 生活を援助するケアマネジメントには､地域的観点を持ち､生活を全体的に捉え､個別的で継続的な視点が求められる｡離島のケアマネジメントの課題は､島内での複数のケアマネージャーの確保であり､｢島外参入型｣は｢島内完結型｣より､介護保険制度のケアマネジメントには課題が多く､克服する努力がより求められると推察された｡|The purpose of this study is to determine the real conditions of the care management for the Yo-Kaigo elderly in isolated islands, and to investigate the tasks of the care management in these islands. The study subjects are the 28 islands that are inhabited and not connected with another island out of the 39 islands which are designated by \"Okinawa Special Measures for Promotion and Development\". The study design is distributing of questionnaires \"The Actual Conditions of Elderly and Infrastructures\", which includes a questionnaire about the distribution of care management, to the all isolated island persons in charge of municipalities. We selected two islands, island A and B, by the types of care management, and we implemented interview and survey of the two islands. Result and conclusions: 1) The way of distribution of care management in 28 islands were divided into 4 types. The types were \"Completed-in-Island Type\" that a care manager living in the island, \"From-out-of-Island Type\" that the care management was from outside of island, \"Usual Type\" that inhabitants could choose care management, or \"Others Type\" that no care management because of no Yo-Kaigo elderly or care managing for oneself. \"From-out-of-Island Type\" was implemented in many little islands that populations were less than 3,000. 2) In the \"Completed-in-Island Type\" island-A, the inhabitants could not select care manager but could use care management whenever they needed. It is because the care manager in island-A was also the nurse in the island clinic that she was expected to fulfill the medical, health and welfare comprehensive needs.3) In the \"From-out-of-Island Type\" island-B, The Long-Term Care Insurance Services were provided beforehand of an assessment and a care management, because of the difficulty of the sea transportation which was influenced by the weather. The services on the \"From-out-of-Island Type\" care management were not based on satisfactory assessments nor advanced plans, at which The Long-Term Care Insurance Systems aim. ４) The community point of view is needed, the comprehension of person's life as a whole, and the individual and continuous view of care giving in a care management for supporting daily life. The task of the care management in isolated islands was to secure more than two care managers. We conjectured the \"From-out-of-Island Type\" islands had more problems to be solved than the \"Completed-in-Island Type\" islands, and need more effort to overcome the
