{"created":"2023-06-19T06:41:45.158463+00:00","id":94,"links":{},"metadata":{"_buckets":{"deposit":"6c79461e-c8e6-450f-aba2-84520b1b4e35"},"_deposit":{"created_by":2,"id":"94","owners":[2],"pid":{"revision_id":0,"type":"depid","value":"94"},"status":"published"},"_oai":{"id":"oai:opcnr.repo.nii.ac.jp:00000094","sets":["1:12"]},"author_link":["184","244","374"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2005-03","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"6","bibliographicPageEnd":"24","bibliographicPageStart":"10","bibliographic_titles":[{"bibliographic_title":"沖縄県立看護大学紀要"},{"bibliographic_title":"Journal of Okinawa Prefectural College of Nursing","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"本研究は、臨地実習における学生のプロセスレコードを分析して、患者-学生間のコミュニケーションの実態と特徴を捉え、今後の実習指導に役立てることを目的としている。研究方法:対象:3年次の学生20名及び臨床指導看護師3名。方法:実習開始第2日目~3日目のプロセスレコードをドキュメントデータとして用いた。分析方法は、社会調査法論の視点から、質的データの解釈方法を用いた。具体的な手続きは次の通りである。1)学生ならびに臨床指導看護師のドキュメントデータのテクストをコード化し、2)学生のコードと臨床指導看護師のそれとを比較し、コードをカテゴリー化する。3)各学生のプロセスレコードをカテゴリーの生起順序とその出現頻度によってコミュニケーションパターンに分類する。結果ならびに考察:1)学生のプロセスレコードは、Sカテゴリー(18コード、表1)とPカテゴリー(19コード、表2)に分類された。Sカテゴリーに含まれたコードは、精神看護のアセスメント力の不足による未熟で、経験の乏しさの見られるコメントやコミュニケーションからなり、Pカテゴリーに含まれたコードは、精神看護のアセスメントと臨床経験に基づく専門的な判断とコミュニケーションが含まれていた。2)各学生のプロセスレコードを、SとPの2つのカテゴリーの出現頻度とその生起順序から分類した結果、S+(P)、S+P,P+S,S→P,P+(S)の5つのコミュニケーションパターンに分類された。それぞれの典型的なコミュニケーションパターンは、図1~5に示した通りである。3)学生の半数(50%)に、未熟な経験の乏しいコミュニケーションパターン(S+(P))が認められ、一方、精神看護のアセスメントに基づいた適切なコミュニケーションパターン(P+(S))の認められたものは、わずかに15%であった。このことから、実習第2、3日という条件を考慮しても、実習における患者-学生間のコミュニケーションは十分なものではないという実態が把握できた。4)本研究の分析結果から、我々は、コミュニケーションスキルと精神看護アセスメントの教育方法について、さらに検討を進める必要があると考える。|In order to know the students' actual communications with mental patients during their psychiatric practice and to utilize the results for developing our educational programs of mental health and psychiatric nursing, we made the sequential content analyses of students' process records. Subjects: Twenty students in the third year in OPCN and three clinical mentors in mental hospital. Method: The students' process records of the second or third day of their practice were used for analyses. 1) Texts of students' and mentors' document data of process record were coded (Table 2) 2) and comparing each student's code with that of mentors, then codes were categorized (Table 3). 3) We finally found out the specific communication pattern in each student's process record by the frequency and sequence of categories. The reliability of the data was established by triangulations through content analyses with authors. Result: 1) Comparing with students' and mentors codes, they were divided into two categories; S category (eighteen codes) and P category (nineteen codes). The codes of S category were included students' immature, inexperienced comments and communications which did not based on the psychiatric nursing assessment, and the codes of P category were appropriate and similar to professional judgements and communications based on psychiatric nursing assessment and experiences with mental patients as shown in Table 2 and 3. 2) By the analyses of frequencies and the sequence of two categories (S and P) in each process record, we divided into five communication patterns such as S (+P), S+P, P+S, S→P, and P (+S). Each typical communication pattern in process record was showed in Figure 1~5. 3) The immature, inexperieced communication pattern S (+P) was observed among 50% of the students.
