| 内容記述 | 【目的】本研究はリンパ浮腫を伴った乳がん患者の日常生活困難感とその対処法および自己との折り合いについて明らかにすることである。【方法】外来通院患者６名を対象に、半構成的面接法とリンパ浮腫に関する生活の質調査（ULL-27 questionnaire：以下ULL-27と示す）を用い記述統計および質的帰納的に分析した。【結果】リンパ浮腫の軽症者は２名、重症者は４名で、ULL-27の平均得点は、軽症者が101.5±17.7点（範囲89-114点)、重症者が74.3±18.2点（範囲49-88点）であった。身体面における日常生活困難感においては、軽症者は、動作に伴う患側上肢のしびれや痛みなどの局所的な症状であり、重症者は、患側上肢の挙上困難や、全身倦怠感など全身的な症状にまで及んでいた。心理社会面の日常生活困難感においては、軽症者、重症者共に、リンパ浮腫悪化の不安や医療従事者によるリンパ浮腫の教育不足がみられていた。リンパ浮腫以外のことで日常生活に困難を感じていることにおいては、乳がん再発後の治療の苦しみなど再発に関するものであった。対処法においては、軽症者、重症者共に浮腫の上肢の挙上と、浮腫を軽減するなどの対処法を行っていた。このような状況のなかで患者は、身体、心理社会的な困難に対処しようと努力し、日々折り合いをつけて生活していた。【結論】看護師は、軽症、重症の程度に伴う困難感の相違に合わせた継続的な教育、患者が選択した対処方法が適切であるかの検討などが必要である。また、さまざまな困難感に直面しながらも、リンパ浮腫と共存した日常生活を送ることが出来るよう患者と看護師が共に考えていけるようなサポート体制の構築が必要である。|【Objectives】The purpose of this study was to investigate the coping behaviors on physical and psychosocial distress of daily living among breast cancer patients with lymphedema. 【Methods】Six breast cancer patients were given a semi-structural interview and a questionnaire for lymphedema at the outpatients ward. Data were analyzed using descriptive statistics and qualitative and inductive methods. 【Results】The results were two Mild Lymphedema Patients (MLP) and four Severe Lymphedema Patients (SLP). The MLP of mean (SD, Range) ULL-27 score was 101.5 (17.7, 89-114), the SLP was 74.3 (18.2, 49-88). The MLP had local symptoms such as the numbness or the pain of upper arms on operated side when they move. The SLP had symptoms such as difficulty raising their arm on operated side and general malaise. Both of MLP and SLP had anxiety for getting worse and lacking education for lymphedema on the daily difficulties of psychosocial life. Other daily difficulties were the treatment of distress for recurrent breast cancer. The coping behaviors for MLP and SLP were raising their arms on the lymphedema side and relief of the edema. The patients assumed effort to cope and compromise with physical and psychosocial difficulties on their daily lives. 【Conclusions】The nurses need to pay attention to the continued education of patients according to their level of the mild or severe lymphedema in difficult situations. Also, nurses need to assess their choice of coping behaviors are right or not. It is necessary to make support system that nurses can think together with their patients for living with lympedema. | |||||
