| 内容記述 | 【目的】本研究は、緩和ケア病棟における緩和ケアの質評価ツールSupport Team Assessment Schedule 日本語版(STAS-J)の導入に対するコア看護師の認識を明らかにすることを目的としている。【方法】調査対象はA緩和ケア病棟のSTAS-Jの導入の推進者であるコア（中心）となる看護師7名である。STAS-J学習会の参与観察法と面接法を用いてデータを収集し、質的に分析を行った。【結果】コア看護師は学習会を通じて、STAS-Jを理解し、デスケースやショートカンファレンスにSTAS-Jを活用していった。STAS-J導入に対するコア看護師の認識は、【ケアの質を考える共通のツール】【カンファレンスにおける情報共有とチームアプローチ】【コミュニケーションを介した患者・家族、医療者との関係の保持】【緩和ケアの基本に立ち返るケア行動】【STAS-Jに関する教育の必要性】という5つのカテゴリーが見出された。【結論】コア看護師は、STAS-Jを学ぶことで、緩和ケアの基本的なアセスメントが可能になり、傾聴や寄り添うことの大切さを認識していた。さらに、STAS-Jの項目を活かし、コミュニケーションを意識したケアを実践していた。STAS-Jの導入において、コア看護師を中心とした学習会を行うことは有用であり、事例検討を重ねながら、それぞれの看護師に応じた教育とサポート体制を行う必要がある。|【Purpose】This research aims to clarify core nurses’ perception of the introduction of the Japanese version of the Support Team Assessment Schedule (STAS-J), a quality assessment tool of palliative care in the palliative care unit. 【Method】The research subjects are 7 core leading nurses who promote the introduction of STAS-J into the palliative care unit. Data was collected by using participant observation and interviews during the STAS-J learning seminar, followed by qualitative analysis of the data. 【Result】Core nurses understood STAS-J through the learning seminar and utilized STAS-J in death cases and short conferences. Core nurses’ perception of the introduction of STAS-J could be classified into 5 categories: 【a common tool to consider the quality of care】【information sharing and team approach in conferences】【retention of relationships between patients, families and doctors through communication】【care action going back to the basics of palliative care】【necessity of education relevant to STAS-J】. 【Conclusion】The study of STAS-J enabled core nurses to conduct basic assessment of palliative care and made them aware of the importance of listening to and taking the part of patients. Moreover, they practiced care focusing on communication by utilizing the items of STAS-J. Holding learning seminar led by core nurses in introducing STAS-J is useful. Education and the support system appropriate to each nurse need to be provided while continuously considering anecdotes. | |||||
