| 内容記述 | 【目的】１人当たり老人医療高騰の要因分析を、県内で１人当たり老人医療費の高い市と低い町の2市町における2003年5月の老人医療入院レセプトと保健事業実績の比較から考察した。【方法】以下の項目の基本統計解析を実施した。(1)2003年5月分老人入院医療レセプトの性、年齢、入院医療費、主傷病名、入院日数、医療機関とその所在地。(2)上記レセプト調査者の要介護度。(3)2000～2002年度の老人保健事業の実績。(4)高額入院医療を、１人当り入院診療費の平均値＋標準偏差の777.1千円とし、その主傷病。【結果】1）両市町の入院者の平均年齢、性別割合、平均入院日数には有意な差が認められなかったが、Ａ市はＢ町より高額入院医療の割合が高かった。その主傷病名は17傷病に分類され、Ａ市では慢性腎炎、肺炎、骨折、慢性閉塞性肺疾患が半数を占め、Ｂ町では骨折および損傷が5件中2件であった。2）Ａ市はＢ町より要介護認定を受けていない、または要介護度が軽度の割合が高かった。3）Ａ市はＢ町より居住地の近くに入院可能な医療機関が多かった。4）Ａ市はＢ町より健康相談以外の老人保健事業の利用率が低くかった。【結論】１人当たり老人入院医療費を高める要因は、入院者に占める高額医療患者の割合であり、その抑制対策には、高額医療のリスクが高い傷病、すなわち慢性腎不全、肺炎、骨折、慢性閉塞性肺疾患、新生物の重症化予防対策に加え、高齢者の健康度を保持増進する保健事業や介護予防事業の強化が必要である。|In order to know the factors related to the increased medical expenses of elderly, the medical fee bills of elderly inpatients and the actual condition of the health services for elderly in two municipalities, Okinawa Prefecture, were investigated. A city where great medical expenses have to be paid and B town where medical expenses are much less than those of A city, were selected to examine the factors. Medical fee bills of 666 elderly inpatients in may 2003 and the implementation status of health services for elderly between 2000 and 2002 were analyzed compared A city with B town. There was no significant difference in the average age, rate of sex, average patient-days of elderly inpatients between A city and B town. What was the difference was that medical expenses of elderly in A city were greater than B town. In A city, there were more hospitals and the distance from hospitals to each inpatient’s home was shorter than B town, while the utilization rate of the health services for elderly in B town was higher than A city. The utilization rate of the health services seems to have some relation with the increased medical expenses. In A city where the utilization rate was lower, there were much more elderly people suffering from chronic or intractable diseases such as chronic renal failure, pneumonia, fractures, chronic obstructive pulmonary disease, neoplasm and they have to pay great medical expenses. On the basis of these facts, this paper researches on the factors related to the increased medical expense. | |||||
