| 内容記述 | 2000年４月から施行された介護保険法は、医療保険制度導入時と同様、離島において「保険あってサービスなし」の事態が危惧された。沖縄県は沖縄県立看護大学と共同で、離島・過疎地域の住民が安心して生活し続けられるような住民主体の地域づくりを支援するため、島嶼県沖縄の中でも特に交通の利便性や高齢化率等の課題が多いＨ島をモデル地区として、平成12年度から５ヶ年計画で「離島・過疎地域支援事業」を開始した。本研究の目的は、介護保険開始直後より２年間追跡した在宅要介護高齢者の介護保険サービスの実態を明らかにし、今後の地域ケアシステムのあり方を検討する際の基礎資料の一つとすることである。対象は、住民票基本台帳がＨ島にあり、かつ要介護認定時島内に在住していた要介護高齢者28名。方法は対象および家族、島内在住のケアマネジャー、行政の介護保険担当者等からの情報収集、および面接聞き取り調査を行い分析した。その結果、平成14年３月末現在で島内に在住している要介護高齢者20例、施設入所中6例、死亡1例、他市町村への転出1例であった。島内で供給可能な在宅サービスは、２年間変化がなく、訪問介護、訪問看護、訪問リハビリ、居宅療養管理指導、福祉用具の貸与・購入、住宅改修であり、通所系や短期入所系サービスはなかった。在宅サービスの利用割合は、沖縄県は全国一利用割合が高いが、Ｈ島においては著しく低かった。ほとんどの要介護高齢者が最期まで島で過ごしたいと希望しているにもかかわらず、施設サービスを利用するために島外に出ていかざるを得ない現状があった。このような状況の中、現状打破に向けＴ町と住民の努力により、平成13年度生きがいデイサービスが新規事業として立ち上がり、自立高齢者だけでなく、要介護高齢者も利用していた。今後も島外からの民間事業者参入は困難な状況であり、島内の事業者による「島内完結型」の在宅サービスの基盤整備が求められている。現在供給可能な訪問系サービスの充実に加え、通所系、短期入所系サービスを創意工夫でつくっていくことが重要になってくる。 When the Long-Term Care Insurance System became effective in 2000, the situation in isolated islands, which has no services but insurance payment, caused considerable apprehension. Okinawa Prefecture and Okinawa Prefectural College of Nursing started the five year program, "Isolated Islands and Depopulation Regions Support Project", from 2000. The purpose of this project was building an inhabitant centered community system that can make it possible for the people in isolated islands and depopulated regions to continue life without anxiety. The model island was H Island, which was one of the nine islands in T Municipality. H Island had more problems like inconvenience in transportation and a high rate of elderly residents. The purpose of this study was to make basic data for discussing the ideal community care systems by clarifying the situations of Yo-Kaigo Elders and in-home services, which had been followed up for two years. Subjects are 28 Yo-Kaigo elders who were living in H Island and also had registrations in T Municipality. We interviewed the subjects and gathered information from the care manager in the island and from the person in charge of Long-Term Life Care Insurance in the town office. Results, Yo-Kaigo elders who lived in H Island were 20, and who were admitted in institutions were 6, one was deceased and one moved out. The numbers of offered services in the island were not changed in these two years. The services were home-visit long-term care, home-visit nursing care, home-visiting rehabilitation, home-visit guidance, and rental service of wheel chairs and other equipment and home improvement services. There were no community long-term cares, nor short stay. The ratio of people who used inhome services was very low compared with Okinawa Prefecture. Almost all Yo-Kaigo elders wanted to stay in the island until the end of their lives, but many had to leave the island for the institutional care. Under these conditions, T Municipality started day service for healthy elders. Not only the healthy elders, but also Yo-Kaigo elders attended to the day services. Because the nonofficial undertaking will not be available, to raise persons who can offer the services and to make in-island completion service systems were expected. It will be important to make community long-term cares and short stay services, in addition to enriching the home-visit services. |
